I am experiencing the same issue on:

  • Apple TV 4K 2nd gen running tvOS 16.2 with just the TV speakers
  • my iPhone 13 mini running iOS 16.2 on both the speakers and Airpods

Whilst running on Jellyfin server version 10.8.8. When I switch the audio track in the file, sound works fine with the other track. I can provide logs if it would be helpful (although I haven't seen anything useful in them).

Unfortunately, Dolby TrueHD playback requires licensing (which is one reason it is only available in the Pro version of Infuse, for example). I think the best we can do for now is transcode it server-side to something like FLAC. I'm looking into that approach as I deal with the device profile (#296) to see if it even makes sense given ffmpeg's truehd implementation (vs. transcoding to something like AC3).

Oh you are right. I didn't realise that none of the devices I tried support it natively - error on my part, especially regarding the Apple devices I mentioned :/ I will check whether any of my devices support it and will report back if it works.
